WellSpace Health is pleased to offer a comprehensive benefits program to our employees including health, welfare, retirement and time-off options.

Benefits Eligibility

Employees scheduled to work 30 or more hours a week are eligible for our health and welfare benefits program and our Paid Time Off (PTO)/Paid Sick Leave (PSL) accrual benefit and paid holidays.

Health and Welfare benefits are effective the first of the month following 30 days of employment.

PTO starts accruing the first day of employment but cannot be used until after 90 days of employment. Holiday pay is applicable immediately.

Paid Sick Leave starts accruing the first day of employment. To qualify for PSL, an employee must satisfy a 90-day employment period (similar to introductory period) before taking any sick leave.

Retirement Plan Eligibility

All employees over the age of 18 are eligible to participate in WellSpace Health’s 401(k) Retirement Savings Plan (full-time, part-time, and on call) after completing three months of service.

Employee Assistance Program (EAP)

This program provides you and any member of your household free, confidential counseling and referral services to help with various issues such as stress and anxiety, grief and loss, relationship problems and finding child or elder care.  You can also find resources for legal and financial guidance and much more.  The EAP is an integrated work and life program and provides online mental wellness and resilience hub resources.

Medical Plans

WellSpace Health provides you with the power to take charge of your own health. We have built our health plan with options and provide resources for overall support to help you make decisions. We offer three medical carriers, Kaiser, Western Health Advantage, and Sutter Health Plus, with eight plan choices. Below is a quick overview of each carrier’s plan options.

Western Health Advantage – WHA

Through Western Health Advantage, you have the choice between two HMO plans (Advantage and Premier) and one NationCare PPO plan. Through the Western Health Advantage HMO Medical plans you select a doctor from Western Health Advantage’s network of participating primary care physicians (PCP). Your PCP will direct and coordinate your care.  The network of providers is the same for both plans, but the plans differ in the co-pays that you pay for benefits.  You have access to Mercy Medical Group, Hill Physicians Medical Group, and Woodland Clinic Medical Group, NorthBay HealthCare, to name a few. The PPO plan is only available to employees who live outside the HMO service areas of Kaiser, Western Health Advantage, and Sutter Health Plus.  This plan allows the greatest flexibility. You can choose any provider.  Benefit allowances differ depending on whether the care is provided by an in-network or out-of-network provider.  You will save more money by choosing a provider that is within the network.

Sutter Health Plus – SHP

Through Sutter Health Plus, you have the choice between two HMO plans (Base and Buy-up). Through the Sutter Health Plus HMO Medical plans you select a doctor from Sutter Health Plus’s network of participating primary care physicians (PCP). Your PCP will direct and coordinate your care.  The network of providers is the same for both plans, but the plans differ in the co-pays that you pay for benefits.  You have access to many of the Sutter hospitals, facilities, and physicians and walk-in clinics are conveniently located around the region.  It is important to note that not all Sutter Health-affiliated hospitals, physician organizations and other health care services are in the Sutter Health Plus service area or network.  Be sure to consult Sutter Health Plus’ website for a list of participating providers and locations.

Kaiser Permanente

Through Kaiser Permanente, you have the choice of three HMO Medical plans ($0, $750, $3000). Co-pays, deductibles and out of pocket maximums for each plan are different. If you choose a Kaiser plan, you must use Kaiser facilities and doctors.  You cannot use non-Kaiser providers unless you require emergency care.  To get started under any plan, simply select a doctor at a Kaiser facility near you. The doctor you choose must be a primary care physician (PCP) practicing in Internal Medicine, Family/General Medicine or Pediatric Medicine. Your PCP will refer you to see a specialist when needed. Most Kaiser locations offer multiple services under one roof.  This means you may be able to see your PCP, get an X-ray, visit the lab and fill your prescription all in the same place.

Supplemental Medical – Solutions Plus Gap Plan

The Solutions Plus Gap plan (SIS) is designed to help reduce the cost of your out-of-pocket expenses under the Kaiser, Western Health Advantage, and Sutter Health Plus medical coverage. The Solutions Plus Gap provides payment for covered out-of-pocket expenses you may incur while you are either confined in a hospital or being treated as an out-patient for an injury or illness. The payment can be made to you or directly to the provider or facility for covered deductible and co-insurance costs as long as the medical plan carrier considers the service a covered benefit under your plan. Each of the four plans is associated with a particular medical plan or plans.  If you are interested in enrolling on one of these plans, be sure you select the correct plan for the medical plan you have chosen.

Dental Options

Delta Dental provides WellSpace Health’s dental insurance. Delta Dental is a well-recognized national insurance carrier offering a large national network of dental providers. We offer both a dental DHMO and a dental PPO Plan, allowing employees to choose the dental plan that works best for them.

With the DHMO plan, you must select a primary care dentist from the list of participating dentists or dental groups. You can enjoy negotiated discounts from the network dentists. You pay fixed copays  or no copay for each covered service and there is no deductible or Annual Maximum limit. The DHMO also includes an orthodontia benefit for both children and adults. Keep in mind that Out-of-Network visits are not covered.

With the PPO plan, you have flexibility in your choice of providers allowing you to receive care from any provider. To reduce your out of pocket expenses, however, select a dentist within the PPO network.   The plan includes the following benefits while accessing In-Network services: No charge for Diagnostic and Preventive services with many Basic services covered at 80% and Major services at 50%. The PPO plan includes an orthodontia benefit for both children and adults paid at 50% with a lifetime maximum of $1,500. The plan has a $50 individual and $150 family deductible, and the plan’s calendar year maximum is $1,000 per insured person.

Vision Option

Vision Service Plan (VSP) provides WellSpace Health’s vision insurance. VSP is a premier vision provider with a nationwide network of more than 33,000 providers including independent eye doctors and retail chains. You will have the choice between two plans (B and C). Both plans allow you to obtain an exam, lenses or contacts every 12 months. Plan C allows you to obtain frames every 12 months and Plan B allows you to obtain frames every 24 months. There are some additional lens enhancements available under Plan C as well.

In addition, each plan offers a primary eye care benefit which allows members to seek care from their primary eye care physician versus their medical doctor for various ocular symptoms and conditions such as Pink Eye, Glaucoma or Cataracts. Each primary care visit is subject to a $20 copay.

Flexible Spending Accounts (FSA)

WellSpace Health offers tax-advantaged plans, through Benefit Resource, that allow you to save money when paying for certain out-of-pocket health and dependent care expenses.  The FSAs work like a savings account.  You make an annual election based on your estimated health care and/or your dependent care expenses.  Each pay period, a pre-tax payroll deduction based on your annual election is deposited into your FSA account(s). Keep in mind that it is important to carefully estimate your upcoming expenses; unused balances at the end of the plan year greater than $550.00 will generally be forfeited.

Basic Life and AD&D Benefit

Lincoln Financial provides Basic Life & Accidental Death & Dismemberment (AD&D) coverage to all eligible employees. This is an employer-paid benefit. The coverage amount for the Basic Life is $50,000. In addition, the AD&D benefit is $50,000.  You also have access to value-added benefits such as travel assistance, estate planning and ID theft protection at no cost to you.  Through TravelConnect, you can get support while traveling 100 miles or more form home if you need medical help, evacuations, prescription refills, cash emergencies and much more. Services are provided for both business and leisure travel. The LifeKeys program provides online will and testament preparation services, identity theft resources and beneficiary assistance support as well as grief counseling.

Voluntary Life / AD&D Benefit

Voluntary Life and Accidental Death & Dismemberment is also offered through Lincoln Financial. This program allows employees to purchase additional insurance at their own expense for themselves and their immediate family members at group rates. With this benefit, you can purchase up to $150,000 of additional coverage for yourself. If you purchase employee coverage, your spouse can be insured for up to $30,000. Children can also be covered up to $10,000 for all eligible children at one monthly cost.

Long Term Disability (LTD) Benefit

Lincoln Financial provides Long Term Disability coverage to all eligible employees. This is an employer-paid benefit. LTD insurance replaces a percentage of your income during extended periods of illness or injury that prevent you from performing your regular work. The benefits will coordinate with any other income benefits the insured employee receives. The benefit amount provided by the plan is up to 60% of monthly earnings to a maximum monthly benefit of $10,000

Legal Shield and Identity Theft Shield Voluntary Benefit

This program allows all employees to purchase legal and identity theft protection services for themselves and their family members. With these products you have access to professional legal counsel not only for traditional legal problems, but for everyday events. In addition, Identity Theft Shield provides regular monitoring of your credit report, plus access to professionals to assist you should you encounter identity theft involving your finances, driver’s license, Social Security number, medical benefits or cyber activity.

401(k) Retirement Savings Plan

Achieving financial security for your future is not just a matter of how much you earn, but more importantly, it’s a matter of how much you save. By saving regularly through WellSpace Health’s 401(k) Retirement Savings Plan, even if only a few dollars each payday, you can accumulate more money in a few years than you would think possible.  You are able to make two kinds of Elective Deferrals including Pre-Tax Elective Deferrals or Roth After-Tax Elective Deferrals.

WellSpace Health wants to help you meet your financial goals with this Plan and makes a matching contribution equal to 25% of your compensation deferral amounts up to legal limits imposed by the I.R.S. Matching contributions are posted to your account on a per pay period basis based on your current deferral percentage election in effect on each particular pay date.

Part-time and on call employees interested in enrolling in the 401(k) plan should contact the Human Resources Department to enroll in the plan after eligibility requirements have been met.

WellSpace Health observes the following ten holidays which are paid for eligible employees:

Paid Time Off

PTO is accrued and is intended to strike a balance between assisting employees with their time off needs and ensuring that WellSpace Health has the staff to operate effectively. PTO is designed to provide pay for employees to take time off due to vacation, personal business, and illness (either the employee’s or his/her immediate family members). PTO is accrued on a per pay period basis. Any unused PTO will be paid upon termination at the employee’s usual rate of pay.

WellSpace Health offers four weeks of PTO for full-time employees (40 hours per week). Full-time employees working at least 30 hours per week receive .077 hours of PTO for each eligible hour worked. WellSpace Health’s PTO accrual rates increase over time as higher levels of seniority are met.

Paid Sick Leave

To qualify for Paid Sick Leave an employee must work for the same employer for at least 30 days within a year. PSL starts accruing on the first day of employment and can be used beginning on the 90th day. PSL is in addition to our existing PTO policy for all benefit-eligible employees. The max accrual amount of PSL is 48 hours. The accrual method for all employees is established at a rate of 1 hour of paid sick leave for every 30 hours worked. PSL is accrued on a per pay period basis. Any unused PSL will not be paid out upon termination of employment with the organization. New Hire Enrollment Process

The Human Resources Department conducts semi-monthly new hire orientations in which benefits are explained. You will receive an email invitation from Human Resources inviting you to an all-day new hire orientation. It is important that you attend this meeting to learn about WellSpace Health and the benefits available to you and to complete your forms within the new hire enrollment period. Enrollments not received during the new hire enrollment period window will be effective during the next Open Enrollment period which usually occurs in August/September of each year for an October 1st effective date.

Continuing Medical and Dental Education for Providers

WellSpace Health offers professional time off and continuing education allowance for full-time benefit-eligible providers. For information about the policy, contact Human Resources Department.
